Camp U of T Intake & Support 2017

Camp U of T strives to make camp an enjoyable experience for all campers. Campers are expected to be able to participate in the full camp experience with minimal adaptation or an inclusion aide may be required. Please note that if abilities are not disclosed at the time of registration, assistance will be limited.
 
For campers ages 4 – 7 staff to camper ratio is 1:6
For campers ages 8 – 16 staff to camper ratio is 1:8
 
This form is intended to provide information to staff for any camper who requires support or modified programming due to exceptionalities. Information on this form will be kept in confidence and shared only with staff working with your child as needed.
 
Text questions are not mandatory, please answer those that you feel would help to make your child's summer a success.
